Add a CHANGELOG.md (Keep a Changelog format) for the project.

Covers the 0.7.0 release — the FOSS pivot:

#37 Retire SaaS mode and subscription billing
#38 Retire demo mode
#39 Remove APP_MODE plumbing; single self-hosted mode
#40 Delete frontend-old/ and dead scripts
#41 Adopt AGPL-3.0

A draft already exists in the working tree on release/v0.7 (currently uncommitted).
